I ran into a bug where mp4box -add-item always resulted in a url instead of an embedded item, as if the dref option was always set.

I tracked this down to the fact that release meta->use_dref is never initialized, and so in release builds could be nonzero. I've attached a patch that initializes use_dref in the same block of code where other variables are initialized (though perhaps a memset would be safer). The patch fixed my issue.
